1. Open the “Status” filter on the Safety Monitor.
   
   
2. Select “Show Canceled” and select the “Canceled” Status to show Canceled Scheduled Forms.
   
3. Select the "Canceled" button next to the Form in question.
   
4. Select "Show Details" to see information about who canceled the Form and when.
