Changing the Form Template Name

Many Analytics Dashboards have a "Form Template Name" filter on them. This is to make sure that your Dashboard is pulling information from the correct Forms(s).

Form Template Name filters use the Title of your Form to filter, so if anything in those words changes even slightly, the filter will no longer be able to find your Form. This can result in Error Codes or "No Results" messages.

If this happens on your Dashboard, simply update the Form Template Name filter to include the new name(s) you've chosen. This will start tracking the Form(s) again.

Changing Form Items

What if I change the Form Item question text in the Form Builder?

Your reports will show all historical data and display the most recent text of that question.

  • Protip: Ensure you aren’t completely changing the nature of the question.
Ex: Don’t change a question from “Are you wearing a hard hat” to “Were you injured today?”. The data will continue to include all the YES answers from when the worker responded that they were indeed wearing a hard hat, but now that the question is “Were you injured?” it will appear like the worker was injured each of those days.

What if I delete a Form Item?

You’ll be able to continue to report on any previous answers for that question, but no future answers will populate. If you recreate the question somewhere else in the Form, you will have to add the new question to your report(s).

Can I move an item in the form?

Yes, you’ll be able to continue to report on any previous answers for that question as long as you do not delete the item. Physical position on the form does not translate to Analytics.
